2. Share Your Medication List

Before the visit, we’ll ask you to share a current list of prescriptions, over-the-counter medications, vitamins, and supplements. This helps us prepare and make the most of your appointment.

3. Meet With a Consultant Pharmacist

During your one-hour consultation, we’ll talk through your medications, health goals, concerns, side effects, daily routine, and what matters most to you.

4. Receive a Personalized Take-Home Packet

After the visit, you’ll receive a personalized packet that may include an updated medication list, a summary of key findings, questions to ask your doctors, and helpful education tailored to your needs.

5. Communicate With Your Care Team

We also provide written information for your physicians or other healthcare providers so you have clear, organized next steps.
