Scheduling Your Personalized Medication Review

At Wright Geriatric Consulting, we provide personalized concierge medication reviews for older adults and caregivers throughout Texas. Our board-certified geriatric pharmacist offers comprehensive medication reviews designed to improve medication safety, simplify medication routines, and support healthy aging through the nationally recognized 4Ms of Age-Friendly Care.

Before You Schedule

Before requesting your consultation, please note:

  • Private-pay service: Concierge medication reviews are private-pay services and are not billed to Medicare, Medicaid, or private insurance.

  • Service area: In-home consultations are available throughout the Greater Houston area. Virtual medication reviews are available anywhere in Texas and New Mexico.

  • Travel: In-home consultations beyond 15 miles from our office may be subject to an additional travel fee.

  • Scheduling: Appointments are typically scheduled 2–3 weeks in advance. After we receive your request, we'll contact you to coordinate the next available appointment.

  • Medicare Part D MTM: This service is separate from the Medicare Part D Medication Therapy Management (MTM) program. If you qualify for a complimentary MTM review through your Medicare prescription drug plan, we encourage you to contact your plan to learn about your available benefits.

What to Expect

Step 1: Request a Consultation

Complete our consultation request form or call us at 281-713-5239. We'll review your request and contact you to schedule your appointment.

Step 2: Prepare for Your Visit

Before your appointment, we'll ask you to share a current list of your prescription medications, over-the-counter medications, vitamins, and supplements. This allows us to prepare personalized recommendations before we meet.

Step 3: Meet With Your Board-Certified Geriatric Pharmacist

Choose the option that best fits your needs:

  • In-home consultation (Greater Houston area)

  • Virtual consultation (Available throughout Texas)

During your one-hour consultation, we'll review every medication, discuss your health goals and concerns, identify opportunities to improve medication safety, and answer your questions.

Every consultation incorporates the 4Ms of Age-Friendly Care:

  • What Matters – Your personal health goals, priorities, and daily routines.

  • Medication – Ensuring every medication is safe, effective, and appropriate.

  • Mentation – Considering how medications may affect memory, mood, and thinking.

  • Mobility – Evaluating whether medications may impact balance, strength, or independence.

Step 4: Receive Your Personalized Medication Plan

Following your consultation, you'll receive a personalized packet that may include:

  • An updated medication list

  • A summary of key findings

  • Personalized medication recommendations

  • Questions to discuss with your healthcare providers

  • Educational resources tailored to your needs

Step 5: Support Your Healthcare Team

When appropriate, we provide written recommendations that you can share with your physicians or other healthcare providers to encourage coordinated, age-friendly care.
